What is Service Navigation?
Service Navigation works together in collaboration with families and professionals by supporting the referral process to access Grandview services. Our team connects clients and families with programs and services at Grandview, as well as resources in the community. Eligibility for services is identified based on an established set of criteria.
Who is eligible for Service Navigation?
Any family living in the Durham Region that is looking for information about Grandview services, is interested in initiating a referral for therapeutic support, or is enquiring about eligibility criteria is encouraged to call Service Navigation. Service Navigation offers a “No Closed Door” policy; our team will do our best to connect families with appropriate services to meet their child’s needs. See the referral process below for details.
What to expect
The Service Navigation Team collects client information based on demographics, developmental milestones, and parent report through telephone intake interviews. Service Navigators will discuss eligibility requirements, anticipated timelines, and answer questions about Grandview services based on the information shared concerning your child’s needs.
Grandview recognizes the diversity of the Durham Region. Service Navigation is available in English and French and interpretation in other languages can also be arranged to assist families who do not speak English or French.

How to make a referral:
- Call (905) 728-1673 x2468 to speak with members of our Service Navigation Team
- Physicians and medical professionals should use the online referral form to make a referral. Completed referral forms can be submitted via fax or mail.

Once the completed referral form is received, families will be contacted and a telephone intake interview will be scheduled.

Changes to Primary Contact Information:
To update your primary contact information, or notify Grandview of a change in address, please call 905-728-1673 x2468, or email email@example.com. If moving outside of Durham Region, a Service Navigator will assist with a transfer of services.
Transfer of Services:
If you are moving in or out of the Durham Region, a transfer of current services, or referral for additional services, can be supported by our Service Navigation team. Please call 905-728-1673 x2468, or email Service Navigation at firstname.lastname@example.org to connect with a Service Navigator.
