What is Service Navigation?
Service Navigation supports families and professionals with the referral process to access Grandview Kids services. Eligibility for services is identified based on an established set of criteria (learn more in the “Who is eligible for Service Navigation?” section below).
In addition to connecting clients and families with programs and services at Grandview Kids, the Service Navigation Team can provide them with helpful resources in the community they may access.
Who is eligible for Service Navigation?
Any family in the Durham Region looking for information about Grandview Kids’ services is interested in initiating a referral for therapeutic support, or is inquiring about eligibility criteria, is encouraged to submit an online referral or question via our electronic referral form. If you are unsure if you need a referral, but just have a question, then please also submit your question via the referral form. Service Navigation offers a “No Closed Door” policy; our team will do our best to connect families with appropriate services to meet their child’s needs. See the referral process below for details.

What to expect
The Service Navigation team collects client information based on demographics, developmental milestones, and parent reports through telephone intake interviews. Service Navigators will discuss eligibility requirements and anticipated timelines and answer questions about Grandview Kids’ services based on the information shared concerning your child’s needs.
Grandview Kids recognizes the diversity of the Durham Region. Service Navigation is available in English and French, and interpretation in other languages can also be arranged to assist families who do not speak English or French.

How to make a referral
Caregivers, clients (youth), physicians and school partners can make online referrals or ask for more information about services via our Referral Form. Click this page and select the referral approach that best matches your needs: refer yourself, refer a child or child, referrals from a physician/health care providers, or referrals from a school partner.

Once the completed referral form is received, families will be contacted, and a telephone intake interview will be scheduled.
Changes to Primary Contact Information
To update your primary contact information or notify Grandview Kids of a change in address, please call the Service Navigation team at 905-728-1673 or toll-free at 1-800-304-6180 (press “4” to connect with the team). If moving outside of Durham Region, a Service Navigator will assist with a transfer of services.
Transfer of Services
If you are moving in or out of the Durham Region, a transfer of current services, or referral for additional services, can be supported by our Service Navigation team. Please call 905-728-1673 or toll-free at 1-800-304-6180 (press “4” to connect with the team) to connect with a Service Navigator.
