2. Did your child or the caregiver attending with the child have a confirmed case of COVID -19 or had close contact with a confirmed case of COVID -19 without wearing Personal Protective Equipment (PPE)?

3. Does your child or the caregiver attending with the child have any of the following symptoms?
- Fever
- New onset of cough
- Worsening Chronic Cough
- Shortness of Breathing
- Sore Throat
- Difficulty Swallowing
- Decrease or loss of sense of taste
- or smell
- Rashes, red swollen toes in children
- Chills
- Headaches
- Unexplained fatigue/ malaise/muscle-aches
- Nausea / Vomiting, diarrhea, abdominal pain
- Pink eye (conjunctivitis)
- Runny nose/nasal congestion without other known cause

4. If the caregiver attending with the child is 70 years of age or older: Are they experiencing any of the following symptoms: delirium, unexplained or increased number of falls, acute functional decline or worsening of chronic conditions?

If response to ALL of the screening questions are NO: COVID Screen Negative – ENTER, practice physical distancing and approach sign in table when space is available.
If response to ANY of the screening questions is YES: COVID Screen Positive – DO NOT ENTER and call for further direction and follow up to either of the following:
