The Durham Region Complex Care Program supports children and youth with complex medical needs, closer to home. This program is delivered in partnership between The Hospital for Sick Children (Sick Kids), Grandview Kids, Lakeridge Health, Central East Home and Community Care Support Services and Complex Care for Kids Ontario (CCKO).
Clients and caregivers benefit from the care, support and expertise of an interprofessional team, inclusive of Paediatrician, Nurse Practitioner, Care Coordinator and Family Engagement Program Assistant. They may also work with other clinicians, such as Physiotherapist, Occupational Therapist, Recreation Therapist, Social Worker and Dietitian.
The program is partially funded through the Durham Ontario Health Team (OHT), a new provincial model of organizing and delivering health care to better connect patients and providers in their communities. The Durham OHT prioritized ‘older adults living with fragility’ and ‘people living with complexity’ as its target populations. Children and youth living with complex medical needs, medical fragility and dependence on technology fall into the second category.
Physician referral is required. Sick Kids determines eligibility.
